Job summary

Four Pillars Hospice in Texas seeks a Nurse Practitioner to oversee clinical care, consult with attending physicians, and provide direction to the interdisciplinary team to ensure quality end-of-life care.

The role Collaborates on patient/family plans, educates families, performs visits, coordinates with physicians, offers on-call coverage, and stays current in pharmacology and palliation.

Qualifications

• Graduate from a Baccalaureate School of Nursing and licensed as RN; NP-focused graduate program required.
• Two years of Advanced Practice Nurse experience preferred; hospice/home health preferred.
• Strong pharmacology, palliative care, and end-of-life knowledge with good interpersonal skills.

Responsibilities

• Collaborate with IDG/T to assure appropriate evaluation and palliative treatment of terminal disease.
• Develop patient/family plan of care with IDG/T to assure quality care.
• Educate patient and family regarding disease, self-care, and end-of-life needs; train staff as needed.
