Qualifications

• Current Ohio RN license.

• Minimum 2 years in Hospice Palliative Care.

Responsibilities

• Lead a team of Hospice Palliative Care RNs in Toledo.
• Develop and implement personalized care plans.
• Provide nursing assessment, intervention, and education.
• Manage transitions and continuity of care.
• Conduct initial and ongoing assessments of physical, emotional, and spiritual needs.
• Mentor junior staff and participate in quality improvement.
