Job summary

Interim HealthCare Hospice in Rock Hill, SC seeks 1–2 registered nurses for on-call rotations to manage after-hours and weekend calls, admissions, and end-of-life visits. The role involves patient assessments, care planning with the IDG, symptom control, and education for patients and families.

The RN will coordinate with IDG members, ensure medication administration, and maintain thorough documentation while supporting quality patient care in home and inpatient settings.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the state of employment.
• 12 months RN experience or current clinical knowledge as deemed by DHCS.
• One year hospice care experience preferred.

Responsibilities

• Assess patient/family needs and plan care with IDG and physician.
• Coordinate care among IDG members and outside providers.
• Administer medications and treatments per physician orders.
• Ensure availability of equipment and supplies.
• Provide education to patient/family on pain and symptom management.
• Supervise LPN/LVN, Hospice Aide, and Homemaker assignments.
• Document services accurately and timely.
• Participate in on-call duties and QAPI activities.
