Job summary

Cherish Hospice Corp is seeking an On-Call Registered Nurse (RN) to cover Warsaw, IN and surrounding areas. The role involves responding to calls, visiting patients, and initiating admissions as required.

The RN will assess patients’ physical and mental health, develop tailored care plans, and coordinate with families and other health care professionals. Travel to patient residences and facilities is expected to ensure quality end-of-life care.

Qualifications

• RN License required.
• Driver's License required.
• BLS Certification preferred.
• Hospice or Home Health experience (1 year) preferred.

Responsibilities

• Cover incoming calls and arrange patient visits and admissions.
• Assess patients' physical/mental health to tailor care plans.
• Collaborate with clients, families, and health professionals to implement care plans.
• Listen to patient concerns and provide counseling or intervention as needed.
• Record patient progress including referrals and home visits.
• Travel to patients' residences and facilities to provide care.

Cherish Hospice is seeking a On Call Registered Nurse (RN) in Warsaw and surrounding areas.

Responsibilities of an On Call RN:

• Covering incoming calls and making necessary patient visits and admissions




• Assessing clients' physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans




• Working with clients, family and friend support networks and health care professionals to put care plans in place




• Listening to clients' concerns and providing counseling or intervention as required




• Recording clients' progress including charting referrals, home visits and other notable interactions




• Travel as necessary to service patients in their residence of care, which includes home, assisted living facilities, long term care facilities, and hospitals.

Qualifications for a Registered Nurse (RN) Include, but are not limited to:

• RN License (Required)
