Job summary

Cherish-Hospice is seeking an On-Call Registered Nurse (RN) to cover Kendallville and surrounding areas. The role involves responding to calls, visiting patients, and coordinating admissions and care plans.

Requirements include an active RN license and driver’s license; BLS certification and hospice/home health experience are preferred. Travel is required as part of the role.

Qualifications

• RN License is required.
• Driver's License is required.
• BLS Certification is preferred.
• Hospice or Home Health experience (1 year) preferred.

Responsibilities

• Covering incoming calls and making necessary patient visits and admissions
• Assessing clients' physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans
• Working with clients, family and friend support networks and health care professionals to put care plans in place
• Listening to clients' concerns and providing counseling or intervention as required
• Recording clients' progress including charting referrals, home visits and other notable interactions
• Travel as necessary to service patients in their residence of care, including home, assisted living facilities, long term care facilities, and hospitals
