Job summary

Cherish-Hospice in Kendallville, IN is seeking an On Call Registered Nurse (RN) to provide compassionate in-home and facility-based care. The role covers calls, admissions, and ongoing patient visits in Kendallville and surrounding areas.

You will assess patient needs, develop care plans with families and healthcare teams, and document progress while traveling to residences and care facilities. An active RN license and driver's license are required, with BLS preferred.

Qualifications

• BLS certification preferred.
• Hospice or Home Health: 1 year preferred.

• Covering incoming calls and making necessary patient visits and admissions
• Assessing clients' physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans
• Working with clients, family and friend support networks and health care professionals to put care plans in place
• Listening to clients' concerns and providing counseling or intervention as required
• Recording clients' progress including charting referrals, home visits and other notable interactions
• Travel as necessary to service patients in their residence of care, which includes home, assisted living facilities, long term care facilities, and hospitals.
