Job summary

Three Oaks Hospice is seeking a Weekend On-Call Registered Nurse (RN) to join our compassionate hospice team. You will provide skilled nursing care to patients in their homes on weekends and coordinate with the care team to ensure seamless, patient-centered support.

The role requires an active RN license, current CPR certification, and a valid driver’s license. You will work within an interdisciplinary team to manage care plans and communicate with families with empathy and professionalism.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and current state driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; MS Office experience preferred.
• Strong interpersonal skills and ability to support patient and family.
• Ability to cope with family emotional stress and diverse lifestyles.

Responsibilities

• Administer skilled nursing care in clients' residences.

We are looking for a passionate On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.
