Job summary

Sage Hospice in Chandler, AZ is seeking a compassionate PRN On-Call Registered Nurse to join our hospice care team. You will provide skilled nursing care in patients' homes, coordinate with the interdisciplinary team, and support families with empathy.

Candidates should hold an accredited nursing degree, an active RN license in Arizona, CPR certification, and be able to work on-call hours in the East Valley region. Experience in hospice or home health is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver's license.
• Two years' RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patient and family.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team and referring agency.
• Collaborate with patient/family to plan and adjust care as needed.
• Provide compassionate education and support to families.

Position Summary

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

Qualifications

• Graduate of an accredited Associate or bachelor's degree in Nursing
• Current state license as an RN and current state driver's license
• Two years' experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
