Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to provide compassionate skilled nursing care to patients in their home. The role emphasizes listening, comforting and coordinating with families to deliver high-quality hospice services.

You will work with an interdisciplinary team to assess needs, develop plans, and ensure seamless patient transitions. A strong clinical foundation, CPR certification and a patient-centered approach are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.
• Strong interpersonal skills to support patients and families.

Responsibilities

• Administer skilled nursing care for patients in their home.
• Coordinate care with interdisciplinary team, patient/family and referring agency.

POSITION SUMMARY

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence coordinate care with the interdisciplinary team patientfamily and referring agency; and assumes the responsibility for coordination of care

QUALIFICATIONS

• Graduate of an accredited Associate or bachelors degree in Nursing


• Current state license as an RN and current state drivers license


• Two years experience as an RN in a clinical care setting; hospice or home health experience preferred


• Current CPR certification


• Proficient skill using a computer is required; Microsoft office suite experience is preferred


• Strong interpersonal skills and ability to provide effective support to the patient and family


• Demonstrate skill in nursing practice ability to cope with family emotional stress and tolerance of individual lifestyles
