Hospice On-Call Registered Nurse – PRN

Advantage Home Health and Hospice is seeking a Registered Nurse (RN) to join our hospice team in Pottsville and surrounding areas. This role offers the chance to directly provide patient‑centered care in the home that focuses on comfort, dignity, and support during life's most challenging moments.

Weekend/Evening – PRN/On‑Call

Responsibilities

• Conducts start‑of‑care comprehensive assessments.
• Dresses care plans, reconciles patient medications, and ensures orders for care are accurate and complete.
• Prepares accurate, orderly, and timely documentation.
• Coordinates care with patient and provides proper reports to Case Manager.
• Provides health‑care instructions to the patient/family as appropriate per assessment and plan.

Qualifications

• An active RN license in the state in which practicing.
• Minimum of 1 year of experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic approach to patient care.
• Ability to work independently and as part of a team.
• Knowledge of hospice regulations and standards of care.
• Reliable transportation and willingness to travel.
