Bloom Healthcare is seeking an On Call Hospice RN to provide after-hours hospice care and coordinate with the interdisciplinary team for patients in the Thornton area. Qualifications

• Graduate of accredited school of nursing.
• Current CO RN/LPN licensure in good standing.
• Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
• Strong clinical assessment and communication skills.
• Proficient computer skills.

Responsibilities

• Provide hospice patient care after-hours as needed for our patients.
• Provide effective pain/symptom assessment and management.
• Provide effective patient/family/caregiver teaching.
• Manage end-of-life ethical issues effectively.
• Create timely and accurate documentation.
• Utilize resources effectively and efficiently.
• Adhere to scope of practice, and continuously improve processes and services.
• Maintain and enhance professional skills.
• Adhere to high standards of personal and professional conduct.
