The role requires RN licensure, 3–5 years in home hospice, strong leadership, and collaboration across clinical teams. Flexible remote options are available; includes a $5,000 hiring bonus and comprehensive benefits.

Qualifications

• Registered nurse with current licensure with the State Board of Nursing.
• Three to five (3 - 5) years of experience in Home Care/Hospice.
• Demonstrated competencies in home care/hospice care management and operations, quality and business management, team building and employee development, client and family support systems, governmental regulations and community outreach.
• Must have a valid driver’s license and proof of auto insurance.
• Demonstrated computer proficiency including Microsoft Office and hospice software programs.

Responsibilities

• Lead a team of RN Case Managers in providing excellent end of life care.
• Guide and mentor the team in a collaborative Christian environment.
• Supervise nursing processes and participate in IDG meetings and compliance.
• Coordinate clinical services with internal and external health professionals to share best practices.
