Qualifications

• Within 30 days: National Provider BLS - American Heart Association
• 3 years clinical experience in chosen field.
• 3 years staff supervision experience (preferred)
• Hospice experience preferred

Responsibilities

• Manage clinical program and supervise and coordinate care for patients and families served by hospice interdisciplinary team.
• Identify best practices for care and improve quality of service.
• Network with community resources and consult with other health care providers serving patients with life-threatening illness.

Education

Bachelor's Degree Health care field preferred
