Job summary

VIA Health Partners in Pineville, NC is seeking a Certified Nursing Assistant with a High School diploma to provide direct patient care in homecare settings. The role emphasizes compassionate, patient-centered care and adherence to care plans.

You will work with an interdisciplinary team, document care per HPCCR guidelines, and participate in ongoing professional development while focusing on comfort and quality of life for terminally ill patients.

Qualifications

• High School diploma preferred, plus training/certification in related field required.
• Certified Nursing Assistant certification and active listing with NC DHHS required.

Responsibilities

• Deliver patient care and personal care responsibilities as an NA (ADL, bathing, mobility, feeding, toileting).
• Monitor and document patient condition and vital signs.
• Participate in interdisciplinary team conferences and care planning.
• Maintain infection control practices and safe handling of patients and equipment.
• Ensure accurate clinical documentation per HPCCR guidelines.
