Job summary

VIA Health Partners is seeking a compassionate Certified Nursing Assistant to provide in-home patient care in North Carolina. You will assist with daily living activities, bathing, meals, and mobility, ensuring safety and comfort, while following care plans and documentation requirements.

This role requires a valid CNA in good standing and at least 1 year of home care or geriatrics experience. Excellent team collaboration, safety awareness, and strong patient-family communication are essential.

Qualifications

• High School diploma preferred; related field training or certification required.
• Certified Nursing Assistant (CNA) in good standing with NC DHHS.

Responsibilities

• Delivery of patient care in home settings.
• Provide personal care including bathing and ADLs.
• Assist with nutrition, meals, and hydration.
• Support with elimination and basic toileting needs.
• Observe safety, infection control, and comfort measures.
• Document care and communicate with interdisciplinary team.

Education
