Job summary

UnitedHealth Group's Heart of Hospice, part of LHC Group, is seeking a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management and supportive care for hospice patients in the Baton Rouge area.

You will conduct face-to-face visits, coordinate plans of care with the Hospice Medical Director and other physicians, educate patients and families, and document care plans and patient responses, while delivering compassionate, high-quality care.

Qualifications

• Active unrestricted nursing license in the state of residence.
• Current certification as Clinical Nurse Specialist or Nurse Practitioner.
• Current national certification such as ANCC, NBCHPN, or AANP.
• Minimum 5 years nursing experience.
• Valid driver’s license and vehicle insurance; access to reliable transportation.

Responsibilities

• Makes face-to-face hospice visits within prescribed time limits and reports findings to the Hospice Medical Director.
• Consults with the Hospice Medical Director and coordinates with primary physicians and other physicians as needed.
• Initiates and coordinates the plan of care; documents problems, goals, interventions, and family responses.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and reports patient status to the Administrator and Hospice Medical Director.
