Job summary

Amedisys in the United States is seeking a Hospice Nurse Practitioner for a PRN role to provide compassionate, high-quality care in patients' homes and collaborate with an interdisciplinary team.

In this position you will evaluate patients, manage palliation, order and review diagnostic tests as needed, coordinate referrals, and participate in team conferences. Flexible hours and mileage reimbursement accompany hourly pay, plus a 401k with match.

Qualifications

• Must hold a current unencumbered Nurse Practitioner license.
• Minimum 1 year of NP or hospice RN experience in the last 3 years.
• Authority to practice and prescribe as NP in the service state.
• Medicare billing privileges for NP required.
• Must meet state NP collaborative requirements; provide agreements if required.
• CPR certification and state insurance as needed.

Responsibilities

• Communicate significant findings and changes in condition to the clinical team.
• Complete face-to-face encounters and assess disease changes, function, pain control, and QoL.
• Order/review labs, imaging, EKGs and other tests for palliation and document plans.
• Make referrals to appropriate disciplines as indicated by patient needs.
• Actively participate in interdisciplinary team conferences.
