Job summary

Central Vermont Home Health & Hospice Inc. is seeking a Nurse Practitioner to provide compassionate, patient-centered home health and hospice care. You will conduct face-to-face encounters, document findings, and support a patient-centered plan of care in partnership with physicians and the care team.

Qualifications

• Graduate of an approved Nurse Practitioner program.
• Active APRN or NP license required.
• Minimum of two years of clinical nursing experience.
• Valid driver's license.

Responsibilities

• Conduct face-to-face NP visits in home care and hospice settings.
• Document findings and coordinate with certifying physicians.
• Provide symptom management and well-patient checks.
• Collaborate with IDT members and community physicians.
• Develop and revise Plans of Care for assigned patients.
• Support Hospice Medical Director and Clinical Manager as needed.
• Attend IDT meetings and provide continuity of care.
