Job summary

Beacon Hospice, Amedisys is seeking a Nurse Practitioner to provide hospice care in New London County, CT. The NP will conduct face-to-face assessments, manage palliative care plans, and collaborate with an interdisciplinary team to optimize patient quality of life at home.

Candidates should hold an active NP license, Medicare billing privileges, and be able to practice and prescribe in the state. Prior hospice or relevant experience is preferred.

Qualifications

• Current and unencumbered Nurse Practitioner license in the state of assignment.
• Minimum 1 year of NP experience or 1 year as hospice RN in last 3 years.
• Authority to practice as NP and to prescribe in the state of service.
• Must have Medicare billing privileges.
• Must meet state-specific physician-nurse practitioner collaborative requirements.
• Insurance as required by state law for NP practice.
• Knowledge of Medicare reimbursement and coding for home/hospice services.
• Current CPR certification.

Responsibilities

• Communicate significant findings and changes in condition to the clinical manager and team.
• Complete face-to-face encounters and assess disease progression, function decline, and quality of life.
• Order and review labs, imaging, and diagnostics; document treatment plan and review results with the physician.
• Make referrals to other disciplines as needed.
• Actively participate in interdisciplinary team conferences.
