Job summary

Enhabit Home Health & Hospice is seeking a Nurse Practitioner to provide medical care to hospice patients and collaborate as part of the interdisciplinary team.

The ideal candidate is licensed as an NP in the state, with knowledge of Medicare conditions of participation, and strong technology skills including mobile devices. Five years of nursing experience with hospice is preferred; a valid driver's license is required.

Qualifications

• Must be a graduate of an approved nursing program and licensed as a Nurse Practitioner in the practicing state.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Education and experience, preferred: at least five years nursing experience with hospice, and at least three years of clinical NP experience.

Responsibilities

• Provide medical care to hospice patients as part of the interdisciplinary team.
• Determine hospice eligibility and pain and symptom management.
• Participate in the care of the hospice patient as a member of the interdisciplinary team.
