Job summary

CorsoCare Hospice seeks a Nurse Practitioner to perform face-to-face assessments of hospice patients across multiple settings, coordinating with the Physician and care team to assess eligibility and provide pain and symptom management. The NP works under supervision of the Hospice Physician or Medical Director.

The role requires a Master's in nursing with NP licensure in the practicing state, and at least 5 years nursing experience; oncology hospice geriatrics and ICU experience preferred.

Qualifications

• Master's degree in nursing.
• Nurse Practitioner certification/licensure in practicing state (DEA license in state).
• Minimum of 5 years nursing experience; oncology hospice geriatrics or ICU preferred.

Responsibilities

• Accountable for an assigned caseload of patients to a developed plan of care, ensuring HIPAA compliance and coordinated with physicians.
• Develops and implements a comprehensive patient-centered care plan and updates per physician orders.
