Qualifications

• Graduate of an accredited School of Nursing and Nurse Practitioner program.
• Minimum of one year NP experience or one year of RN hospice experience.
• Current nursing license in the state of practice.

Responsibilities

• Provide compassionate medical care and symptom management.
• Perform assessments and develop care plans.
• Conduct and document face-to-face visits for eligibility.
