Job summary

Interim HealthCare Hospice & Palliative Care seeks a Hospice and Palliative Care Nurse Practitioner (NP) to deliver compassionate care when you’re needed most. You will perform bedside evaluations, assist with diagnosis and management, and collaborate with palliative care physicians to support families.

Licensure as an Advanced Practice Nurse and RN in the state is required. We offer competitive pay, flexible PRN scheduling, daily pay option, and a patient‑centred team dedicated to changing

Qualifications

• To qualify you must hold active RN and Advanced Practice Nurse licenses.
• Hospice or Palliative care experience preferred.

Responsibilities

• Perform bedside palliative care evaluation and assist with diagnosis, treatment and management of acute and chronic conditions.
• Consult with palliative care physicians and inform primary physicians of services provided; collaborate with other physicians as needed.
• Prescribe medications to the extent delegated and licensed; prepare and maintain accurate patient records and documentation.

As a Hospice and Palliative Care Nurse Practitioner (NP) you will:

• Perform bedside palliative care patient/family evaluation and assist with diagnosis, treatment and management of acute and chronic health conditions
• Consult with palliative care physician or designees as needed, inform primary physician of services provided and collaborate with other physicians as needed
• Prescribe medications including controlled substances to the extent delegated and licensedPrepare and maintain accurate patient records, charts and documents to support sound medical practice and reimbursement for services provided

To qualify to be Hospice and Palliative Care Nurse Practitioner (NP) with us, you will need:

• Licensure: Current license Advanced Practice Nurse and Registered Nurse (RN) in the state associated with this position.
• Controlled Substance license
• Hospice or Palliative care experience preferred
