Job summary

AHN Healthcare@Home in New Kensington, PA is seeking a Nurse Practitioner to deliver hospice care to patients at home. A $10,000 first-year success bonus rewards milestone achievements during the first year.

The NP will collaborate with an interdisciplinary team, perform comprehensive assessments, manage symptoms and meds, and educate families while maintaining EMR documentation. Travel to patient homes within the territory is required.

Qualifications

• BSN and MSN degrees from accredited schools.
• Minimum of five years of clinical experience.
• RN and NP licenses in the state of work.
• Community-based Home Health or Hospice/Palliative care experience preferred.
• Valid driver’s license and ability to travel ~20% of the workday.

Responsibilities

• Provide hospice care in patients' homes with an interdisciplinary team.
• Conduct comprehensive clinical assessments and manage symptoms/medications with individualized care plans.
• Educate and support patients, families, and caregivers while ensuring accurate EMR documentation.

• Work independently, but never alone

Nurse Practitioner Qualifications

• Bachelor of Science (BSN) degree from an accredited School of Nursing


• Master of Science (MSN) degree from an accredited School of Nursing


• A minimum of five (5) years of clinical experience


• Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work


• Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred


• Due to the field-based nature of this home hospice position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
