Job summary

Bloom Healthcare in Lone Tree, CO is seeking Hospice On Call RNs to provide and coordinate hospice care after-hours in patients' homes, collaborating with the interdisciplinary team to address physical, emotional, and spiritual needs.

The role requires a graduate of an accredited nursing program, current CO RN licensure, and at least one year of medical, surgical, or critical care nursing experience; strong assessment, communication, and computer skills are essential.

Position Profile

On call RN's at Bloom hospice have the primary responsibility to provide and coordinate hospice care for our patients and families during after-hours. They give care and collaborate with the interdisciplinary hospice team to make sure patients' physical, emotional, and spiritual needs are met. On call RN's educate patients and families about the disease process, help manage and control symptoms and offer support and strength during difficult times. Care delivery primarily takes place in the patient's home or in a community where the patient resides and calls home.

Hospice On Call RN Responsibilities

• Provide hospice patient care after-hours as needs arise for our patients
• Provides effective pain/symptom assessment and management
• Provides effective patient/family/caregiver teaching

Hospice On Call RN Minimum Qualifications

• Graduate of accredited school of nursing
• Current CO RN licensure in good standing

• Solution driven, creative and resourceful problem solving skills

Hospice On Call RN Preferred Qualifications

• Previous hospice or oncology experience
• Knowledge of the death/dying and bereavement process
• CHPN certification

The pay range for this role in the state of Colorado typically falls between $75,000 - $100,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
