Qualifications

• One year experience as a professional nurse within the past three years or a Bachelor’s degree in Nursing from an accredited School of Nursing.

Responsibilities

• Assess, plan, implement and evaluate patient/family needs using the nursing process.
• Coordinate all patient/family services and prioritize needs with the transdisciplinary team.
• Use a case management approach and refer to other services as needed.
• Coordinate the implementation of the plan of care and ensure care coordination for each patient.
• Apply admission and re-certification criteria to hospice care to establish appropriate levels of care and patient eligibility.
