Job summary

By The Bay Health, a nonprofit hospice provider serving Marin, Sonoma and Napa counties, seeks an After-Hours Nurse to deliver skilled nursing and hospice care to maximize comfort and health for patients.

This full-time position requires California RN licensure, a minimum of one year nursing experience, and preferred hospice/home health exposure. Responsibilities include assessment, care planning, and coordination with physicians to ensure optimal patient outcomes.

Qualifications

• 1 year of professional nursing experience within last three years or a BSN from an accredited school.
• Experience in acute care, home health or hospice setting preferred.
• Current California RN license.

Responsibilities

• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Under the guidance of the After-Hours Nurse, assess, plan, implement, and evaluate nursing services per the plan of care.
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.
• Maintain regular communication with Team Leaders regarding patient/family status and any issues.

We are seeking an After-Hours Nurse who will provide skilled nursing and hospice care to maximize the comfort and health of patients.

Schedule:

Thursday & Friday; 3:30pm - 12:00am

Saturday & Sunday; 9:00am - 5:30pm

Office Location:

Marin County with additional coverage to Sonoma & Napa Counties as needed

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
• Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.

Education/Experience:

• One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations:

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred

Pay Range:

$64.50 - $78.70 shift differential included
