Job summary

Providence Hospice and Palliative Care in St. John, IN is seeking a Nurse Practitioner (PRN) to assess and manage palliative patients across Lake and Porter Counties.

You will perform initial and follow-up visits, coordinate with MSW and RN staff, conduct interviews, interpret diagnostics, and help determine hospice eligibility. Must hold Indiana NP license and have hospice/home health experience; valid driver's license required.

Qualifications

• Must be a Certified Nurse Practitioner NP for Indiana.
• Minimum 1–2 years of hospice or home health experience preferred.
• Driver's License in good standing.
• Reliable transportation.
• Must pass physical, drug screen, background check and references.

Responsibilities

• Manage care of palliative patients during initial and follow-up visits with MSW and RN staff.
• Conduct patient interviews and interpret diagnostic data.
• Assess disease trajectory to determine hospice eligibility.
• Coordinate care: orders, labs, tests, consults with the hospice Medical Director and team.
• Facilitate communication with patient and family to align end-of-life goals.
• Collaborate with IDG team, social services, and primary care physicians to coordinate plan of care.
