Job summary

Cherish Hospice Corp is seeking a PRN Nurse Practitioner in Warsaw, IN to provide compassionate hospice care on an as-needed basis. You will perform assessments, manage symptoms, prescribe medications, and work with a multidisciplinary team to ensure comfort and dignity for patients at home or in facilities.

The role emphasizes flexible scheduling, strong collaboration, and adherence to hospice standards while supporting families during challenging times.

Qualifications

• Active Nurse Practitioner License (FNP, AGNP, or similar).
• Valid Driver's License.
• Current professional liability insurance.
• 1+ years clinical experience.

Responsibilities

• Provide expert assessments, manage symptoms, and prescribe medications to ensure comfort and quality of life.
• Serve as clinical resource to the interdisciplinary team with advanced knowledge.
• Build trusting relationships with patients and families to support them during difficult times.
• Develop and adjust care plans prioritizing pain management, symptom control, and dignity.
• Collaborate with RNs, LPNs, social workers, and chaplains to deliver holistic care.
• Make clinical decisions during visits and urgent needs to ensure continuity of care.
• Maintain accurate, timely clinical documentation compliant with hospice regulations.
