Job summary

Heart 'n Home Hospice, part of the LHC Group, is seeking a Hospice Nurse Practitioner to provide consultation in palliative and supportive care. You will conduct face-to-face visits, coordinate plans of care, and document patient progress for the Hospice Medical Director and referring physicians.

Ideal candidates hold an unrestricted NP license, current CNS/NP certification, and CPR, with 5 years of nursing experience. A dependable vehicle and valid driver’s license are preferred.

Qualifications

• Active unrestricted license in the state of residence.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner.
• Current certification by ANCC, NBCHPN, or AANP.
• Current CPR certification.
• Minimum 5 years of nursing experience.

Responsibilities

• Makes face-to-face encounter visits within prescribed time limits for Hospice patients.
• Reports clinical findings to the Hospice Medical Director and collaborates with physicians.
• Initiates and coordinates the plan of care.
• Maintains accurate clinical notes and communicates patient needs to the team.
• Supervises patient/family care and provides self-care instruction when appropriate.
