Job summary

Community Healthcare of Texas is seeking a Nurse Practitioner to provide Hospice care to geriatric and pediatric patients in collaboration with the Hospice Team. You will perform procedures within your scope and document assessments and progress notes.

The role requires active Texas APRN licensure, at least 2 years in geriatrics or hospice, and strong organizational and computer skills. This PRN position emphasizes compassionate service aligned with our mission.

Qualifications

• Licensed as an Advanced Practice Nurse in the State of Texas.
• Minimum 2 years experience in geriatrics or hospice care.
• Excellent organizational skills and communication abilities.
• Proficient computer skills and Microsoft Office proficiency required.

Responsibilities

• Conduct Face-to-Face visits to assess hospice eligibility.
• Document assessments, evaluations, and progress notes.
• Communicate patient needs requiring medical intervention to RN Case Manager.
• Coordinate visits and scheduling with the Patient Care Manager.
• Maintain regulatory and accrediting compliance.
