Job summary

The Care Team Hospice in Youngstown, OH is seeking a compassionate Nurse Practitioner to join our interdisciplinary team, providing expert clinical assessments and end-of-life care in patients' homes and care facilities. Qualifications

• Current Indiana NP license and NP accreditation.
• Minimum of two years advanced practice or clinical nursing experience.
• Hospice, palliative care, home health, geriatrics, or primary care experience preferred.

Responsibilities

• Complete Face-to-Face hospice recertification visits per Medicare guidelines.
• Conduct comprehensive clinical assessments and evaluate disease progression.
• Collaborate with physicians, nurses, and other care team members on plans of care.
• Educate patients, families and caregivers on disease progression, symptom management, and end-of-life care.
• Maintain accurate documentation and ensure compliance with hospice regulations.
