Job summary

UnitedHealth Group seeks a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management, and supportive care for hospice patients as requested by the referring physician.

Qualifications

• Active unrestricted nursing license in practicing state.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner.
• Current national certifications (ANCC, NBCHPN or AANP).
• 5+ years of nursing experience.

Responsibilities

• Makes face-to-face encounters with hospice patients within prescribed time limits.
• Consults with Hospice Medical Director, informs primary physician and collaborates with other physicians.
• Initiates and coordinates the plan of care; documents goals, interventions, and patient/family response.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes and communicates patient status to leadership.
