Job summary

Affinity Hospice Management LLC seeks a Music Therapist to provide music therapy services to address physical, emotional, cognitive and social needs of patients. The role may be contracted or employed and requires collaboration with the Clinical Manager and hospice care team.

The therapist will assess patient needs, implement therapy plans, train family/caregivers, and contribute to quality improvement efforts while maintaining CPR certification and state licensure as a music therapist.

Qualifications

• Bachelor's or Master's degree in music therapy from an accredited program.
• Current CPR certification.
• Two years of music therapist experience; hospice/community preferred.
• State licensure as a music therapist.
• Valid driver's license with insured vehicle.

Responsibilities

• Provide music therapy services per written plan of care.
• Assess and evaluate therapeutic status; aid in development of care plan.
• Direct music therapy treatment and set priorities for therapy.
• Report patient response to treatment to physician.
• Prepare and submit clinical/progress summaries.
• Offer training/education to families and staff on therapy goals.
• Participate in interdisciplinary care teams and quality initiatives.
• Complete orientation and ongoing professional development for hospice team.
