Job summary

Affinity Hospice Management LLC is seeking a Music Therapist to address physical, emotional, cognitive and social needs of our patients. The role can be contracted or employed and reports to the Clinical Manager.

The therapist will develop and implement individualized treatment plans, collaborate with an interdisciplinary team, and provide education to families and caregivers while participating in quality improvement activities.

Qualifications

• Minimum 2 years of relevant music therapy experience.
• Current CPR certification.
• Licensed or eligible to practice as a music therapist in the state.
• Baccalaureate or Master’s degree in music therapy.

Responsibilities

• Provide music therapy per an individualized plan of care; assess and evaluate therapeutic status.
• Initiate therapy program and train staff/family on goals of the program.
• Prepare and submit clinical and progress summaries.
• Provide music therapy consultation to families/caregivers.
• Deliver in-service education to nursing personnel as needed.
• Participate in interdisciplinary team.
• Contribute to quality assessment and performance improvement activities.
• Participate in hospice team orientation.
