Job summary

AdvisaCare Home Health and Hospice is seeking a full-time MSW to join our team serving Genesee and Lapeer counties, with Troy as the parent agency. We require an MSW degree from a CSWE-accredited school, along with 1 year of home health care experience preferred and hospice experience preferred.

MSW degree from a CSWE-accredited school.
One year of experience in Home Health Care preferred.
Hospice experience preferred.
Valid Driver's License, car insurance and reliable transportation.

Responsibilities

Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in goal-directed care plans.
Provides care under physician direction; participates in IDT conferences, assessing psychosocial, emotional, spiritual, physical and financial conditions.
Documents all patient/family visits, calls and referrals promptly in the patient record.
Conducts reassessments of patient/family needs and counseling as required.
Provides crisis intervention and individual or family counseling when indicated.
Acts as a care advocate for the patient/family unit.
Maintains contact with community agencies to facilitate referrals.
Provides ongoing counseling related to death and dying as needed.
Participates as a Bereavement Team member as assigned.
Adheres to Hospice policies.
