Job summary

AdvisaCare Home Health and Hospice in Michigan is seeking a Per Diem/ Part time MSW (Masters of Social Work) and Volunteer Coordinator to serve Genesee and Lapeer Counties, with the Troy location as the parent agency.

Responsibilities include psychosocial assessments, bereavement coordination, collaboration with the medical team, and community referrals to support patients and families facing life-limiting illness.

Qualifications

• MSW degree from a CSWE-accredited program.
• One year of experience in Home Health Care preferred.
• Hospice experience preferred.
• Valid driver's license with car insurance and reliable transportation.

Responsibilities

• Performs psychosocial assessments and assists in goal-directed care planning.
• Participates in IDT conferences and communicates psychosocial and family needs.
• Documents visits, calls and referrals accurately in the patient record.
• Provides crisis intervention and counseling as indicated.
• Maintains contact with community agencies to facilitate referrals.
• Offers ongoing counseling related to issues of death and dying.
• Acts as a care advocate for patients and families.
