Job summary

Elara Caring is seeking a Medical Social Worker (MSW) in Kansas to contribute to our mission of delivering the Right Care, at the Right Time, in the Right Place. You will assess patients, develop care plans, and connect families with resources while collaborating with physicians and care teams to ensure safe discharge planning.

The role requires a state license, CSWE-accredited MSW degree, and at least 1 year of healthcare social work experience.

Qualifications

• Current State License as a Social Worker.
• Master’s or Doctoral Degree in Social Work (CSWE-accredited).
• 1+ year of healthcare social work experience.
• Ability to lift up to 50 pounds and travel between patient homes.
• Valid driver’s license or reliable transportation.

Responsibilities

• Assess patients to identify psychosocial, financial, environmental, and safety needs.
• Complete initial social work evaluations and reassess patient needs during follow-up visits.
• Develop and update individualized care plans based on patient needs, goals, and physician orders.
• Communicate changes in patient condition, concerns, or safety issues to physicians and the care team.
• Connect patients and families with appropriate community resources and support services.
• Counsel and educate patients and families to address social, emotional, and care-related needs.
• Coordinate with the care team to support continuity of care and effective discharge planning.
• Provide compassionate, high-quality social work services that support positive patient outcomes.
