Job summary

Interim HealthCare is seeking a Hospice Medical Social Worker to visit patients and families in their homes in El Dorado, KS, providing psychosocial support and resources to help them cope with illness.

You will collaborate with a hospice team, assess needs, document progress, and advocate for patient goals. Must hold an MSW license in Kansas and CPR certification, with home health experience a plus.

Qualifications

• Master’s degree in Social Work is required.
• Active MSW license in Kansas is required.
• CPR certification is preferred.

Responsibilities

• Provide home-based social services to patients and families.
• Work with hospice team to implement plan of care and goals.
• Monitor psychosocial needs and document progress toward goals.
• Conduct patient assessments, counseling, and resource planning.
• Consult with family to support patient progress.
