Job summary

Praire Wind Hospice in the Kansas City area seeks a licensed Social Worker with an MSW from a CSWE-accredited program to join the Interdisciplinary Team.

The successful candidate will assess psychosocial and spiritual needs, evaluate insurance coverage, develop and implement the patient/family Plan of Care, provide counseling to families and staff, and coordinate with other professionals while upholding the Hospice Code of Ethics.

Qualifications

• MSW degree from CSWE-accredited program.
• At least 2 years of experience with geriatric population.
• Two years hospice experience preferred.
• Excellent communication and interpersonal skills.
• Ability to work with diverse healthcare providers and families.

Responsibilities

• Assess psychosocial/spiritual needs of patients and families.
• Evaluate insurance coverage and refer to assistance programs.
• Provide social services related to terminally ill patients and families; plan and consult with staff.
• Develop and implement the patient/family Plan of Care.
• Participate in IDT meetings and keep team informed of psychosocial updates.
• Provide counseling to patients, families, and hospice staff.
• Refer to community resources as appropriate to meet patient/family needs.
• Participate in bereavement follow-up services as required.
• Maintain complete records of activities and services.
