Job summary

Phoenix Home Care & Hospice is seeking a Hospice Social Worker to provide counseling and support to patients and families in the home, coordinating resources and advance directives. The role operates on a PRN basis in Hutchinson, KS and surrounding areas, collaborating with the IDG to ensure comprehensive, compassionate care.

Ideal candidates hold an MSW or LCSW and have hospice experience, with strong communication and organizational skills for independent or team-based work.

Qualifications

Responsibilities

Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.

Requirements

• Master’s in Social Work (MSW) or LCSW required
• Hospice experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
