Job summary

Enhabit Home Health & Hospice is seeking a licensed social worker to administer social services for patients requiring intermittent professional services and to educate patients, families, and the care team in accordance with the physician’s orders and the plan of care.

The role requires a master's degree in social work and current state licensure (LSW/LMSW) with at least one year of health care experience. Alabama, Alaska, and Oklahoma licensure guidelines are specified in the posting.

Qualifications

• Must have a masters degree from a school of social work
• Must be certified or licensed in the state where they currently practice
• Must have at least one year of social work experience in a health care setting
• Must be able to safely drive an automobile in all types of weather conditions.
• Must possess CPR certification for the healthcare provider.

Responsibilities

• Provide social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Work in accordance with the physician’s orders and the established plan of care.
