VitalCaring is seeking a Masters Social Worker (MSW) for Hospice in the Lafayette, LA area. This field-based role supports patients and families navigating terminal illness with counseling, planning, and resources.

You will work within an interdisciplinary team to ensure dignity, comfort, and quality of life during end-of-life care. Candidates should have an MSW and at least 1 year of hospice experience, with strong interpersonal skills and cultural sensitivity.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Experience providing psychosocial support in hospice or end‑of‑life settings preferred.
• Strong communication and interpersonal skills essential.

Responsibilities

• Assess psychosocial needs of patients and families facing terminal illness.
• Provide counseling and emotional support to patients and caregivers.
• Identify risks and offer crisis intervention when needed.
• Guide advance directives and care planning in collaboration with the team.
• Maintain accurate records and communicate with the interdisciplinary team.
