Job summary

Residential Home Health and Hospice is hiring a Medical Social Worker (MSW) for a Hospice position serving patients in their homes around Ann Arbor, MI. The MSW will provide compassionate, patient-centered care and collaborate with an interdisciplinary team to support emotional and end-of-life goals.

The role requires a Master’s in Social Work, an active state license, and 1–2 years of healthcare social work experience. Travel to patient homes is expected within the territory.

Qualifications

• Master’s in Social Work from graduate school accredited by the Council on Social Work
• One to two years post graduate social work experience in a health care setting
• Must hold, in good standing, a license as a Medical Social Worker issued by the state
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Conduct comprehensive psychosocial and bereavement assessments, identifying patient and family needs while developing care plans that support emotional, social, and end-of-life goals
• Provide counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey with clarity and dignity
• Apply critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation
• Work independently but never alone
• Travel to patient homes within assigned territory
