Qualifikationen

• MSW from an accredited program.
• Active Texas LMSW licensure.
• Minimum 1 year social work experience in health care.
• Strong interpersonal skills and knowledge of community resources.
• Pass criminal background checks and TB testing; Hepatitis profile.

Aufgaben

• Comprehensive Assessment: identify psychosocial, financial, and environmental needs to guide the plan of care.
• Clinical Counseling: provide counseling to patients and families, focusing on advanced care planning and grief.
• Interdisciplinary Collaboration: participate in team meetings for seamless care coordination.
• Community Advocacy: leverage community resources and eligibility standards for patients.
• Clinical Documentation: maintain accurate records and meeting minutes.
