Zusammenfassung

Companion Hospice is seeking a compassionate Licensed Clinical Social Worker to join our interdisciplinary team and provide meaningful end-of-life support to patients and families in their homes and local facilities.

You will conduct psychosocial assessments, offer counseling, collaborate on the Plan of Care, and connect families with community resources, while contributing to quality improvement and staff education across Guthrie, Edmond, and Stillwater areas.

Qualifikationen

• MSW from CSWE-accredited program and Oklahoma LCSW license required.
• Minimum one year of social work experience; hospice/home care preferred.
• Knowledge of local resources and strong communication skills highly desirable.
• Valid driver's license and auto insurance required.

• Conduct in-depth psychosocial assessments of patients and families.
• Provide short-term counseling, anticipatory grief support, and crisis intervention.
• Collaborate with the IDG to develop and update the Plan of Care.
• Connect patients with community resources and assist with Medicaid and directives.
• Participate in bereavement support and complete timely documentation.
• Contribute to discharge planning, quality improvement, and staff education.
• Perform on-call duties as needed.

Your Impact as a Hospice Medical Social Worker

• Provide short-term counseling, anticipatory grief support, and crisis intervention
• Collaborate with the IDG to develop and update the Plan of Care
• Connect patients and families with community resources, assist with Medicaid applications, and support advance directives and POA/DPOA issues
• Participate in bereavement support as needed and complete timely, accurate documentation
• Contribute to discharge planning, continuous quality improvement, and staff education
• Perform on-call duties as required

What You’ll Bring

• Master’s degree in Social Work (MSW) from a CSWE-accredited program
• Licensed Clinical Social Worker (LCSW) in Oklahoma (required)
• At least one year of experience as a social worker; hospice or home care experience preferred
• Knowledge of local community resources preferred
• Excellent communication, interpersonal, and organizational skills
• Valid driver’s license and current auto insurance
• Ability to work independently in patients’ homes and community settings while remaining a strong team collaborator

Physical Demands & Work Environment (summary)

This role involves regular travel to patient homes and community locations in varying weather conditions, intermittent standing/walking/stooping, lifting up to 20-25 lbs, and the ability to document and communicate effectively. Reasonable accommodations may be available.
