Companion Healthcare is seeking a compassionate Licensed Clinical Social Worker to join our hospice team in Guthrie, Oklahoma. You will conduct psychosocial assessments, provide counseling, and collaborate with the interdisciplinary group to develop and update the Plan of Care, delivering care in patients' homes and community settings.

This role requires an MSW from a CSWE-accredited program, an LCSW in Oklahoma, at least one year of experience (hospice/home care preferred), and a valid driver’s

• LCSW licensure in Oklahoma required.
• At least 1 year social work experience; hospice or home care preferred.
• Knowledge of local community resources preferred.
• Valid driver’s license and current auto insurance.

Responsabilités

• Conduct in-depth psychosocial assessments for patients and families.
• Provide short-term counseling, anticipatory grief support, and crisis intervention.
• Collaborate with IDG to develop and update the Plan of Care.
• Connect patients and families with community resources and assist with Medicaid applications and advance directives/POA issues.
• Participate in bereavement support and complete timely documentation.
• Contribute to discharge planning, QA, and staff education.
• Perform on-call duties as required.

Your Impact as a Hospice Medical Social Worker

What You’ll Bring

• Master’s degree in Social Work (MSW) from a CSWE-accredited program
• Licensed Clinical Social Worker (LCSW) in Oklahoma (required)
• At least one year of experience as a social worker; hospice or home care experience preferred
• Knowledge of local community resources preferred
• Excellent communication, interpersonal, and organizational skills
• Valid driver’s license and current auto insurance
• Ability to work independently in patients’ homes and community settings while remaining a strong team collaborator

Physical Demands & Work Environment (summary)

This role involves regular travel to patient homes and community locations in varying weather conditions, intermittent standing/walking/stooping, lifting up to 20–25 lbs, and the ability to document and communicate effectively. Reasonable accommodations may be available.
