Companion Hospice in Guthrie, OK seeks a Licensed Clinical Social Worker to join its interdisciplinary team as a Hospice Medical Social Worker. You will conduct psychosocial assessments, provide counseling and crisis support, and collaborate on the Plan of Care to ease the emotional and practical burdens for patients and families in life-limiting illness.

Formación

• Master's degree in Social Work (MSW) from a CSWE-accredited program.
• Licensed Clinical Social Worker (LCSW) in Oklahoma (required).
• At least one year of experience as a social worker; hospice or home care experience preferred.

Responsabilidades

• Conduct in-depth psychosocial assessments to identify emotional, social, financial, and community needs of patients and families.
• Provide short-term counseling, anticipatory grief support, and crisis intervention.
• Collaborate with the IDG to develop and update the Plan of Care.
• Connect patients and families with community resources, assist with Medicaid applications, and support advance directives and POA/DPOA issues.
• Participate in bereavement support as needed and complete timely, accurate documentation.
• Contribute to discharge planning, continuous quality improvement, and staff education.
• Perform on-call duties as required.

What You’ll Bring

Physical Demands & Work Environment (summary)

This role involves regular travel to patient homes and community locations in varying weather conditions, intermittent standing/walking/stooping, lifting up to 20–25 lbs, and the ability to document and communicate effectively. Reasonable accommodations may be available.
