Job summary

St. Joseph Hospice in Covington, LA is seeking a Full-Time Licensed Medical Social Worker to join our patient care team serving hospice patients in the local area.

As part of the team you will complete documentation, conduct patient assessments, and coordinate care with patients, families and co-workers. Strong communication, time management, and a Louisiana MSW license are required; Home Health or Hospice experience is preferred.

Qualifications

• Good oral and written communication skills
• Previous Home Health or Hospice experience preferred

Responsibilities

• Complete documentation accurately and in a timely manner
• Conduct patient assessments and ongoing care of hospice patients
• Communicate with patients, families and co-workers
• Manage casework needs of terminally ill patients and family
• Participate in discharge planning and in-service programs related to the needs of the patient
• Great time management skills
