Elara Caring is seeking a Hospice Medical Social Worker (MSW) in Tinley Park/Des Plaines to assess psychosocial needs, develop care plans, and coordinate with the care team for smooth discharge planning. The role requires an MSW with state licensure and healthcare experience, plus ability to lift up to 50 pounds and travel between patient homes.

Formación

• Current State License as a Social Worker
• MSW or Doctoral degree in Social Work from CSWE-accredited program
• 1+ year of social work experience in a healthcare setting
• Ability to frequently lift up to 50 pounds and travel between patient homes

Responsabilidades

• Assess patients' psychosocial, financial, environmental, and safety needs.
• Develop and update individualized care plans based on patient needs, goals, and physician orders.
• Coordinate with the care team to support continuity of care and discharge planning.
• Connect patients and families with appropriate community resources and support services.
• Counsel and educate patients and families to help address social, emotional, and care-related needs.

Educación

Master’s or Doctoral Degree in Social Work from a CSWE-accredited school
